Instructions for Filling Out the Online Surveys

In addition to your web browser (e.g., Internet Explorer, Firefox, etc.), you will need version 6 or newer of Adobe® Reader® to complete the surveys online. When you click on a hyperlink below, your web browser will load Adobe Reader and the survey. You can then use the mouse and keyboard to complete the forms as for any other online survey.

• If you do not have the latest version of Adobe Reader, download a copy here: http://www.adobe.com/products/acrobat/readstep2.html.
• If you'd rather complete a paper and pencil version of this survey, please click on this e-mail link (alcurric@uwm.edu) and ask that one be sent to you. Be sure to include your mailing address.

• When you first begin entering data into a form, you may receive an error from Adobe Reader telling you that your data will not be saved as a part of the form. Please ignore this warning message. Even though the data are not saved, when you click on the SUBMIT button at the end of each form, the data will be sent to the UWM web server and e-mailed to a special e-mail account CABHR maintains for this purpose.
• After submitting a form you may get a message that indicates a field was left blank. If this happens, simply look over the form, make sure you have answered all questions and resubmit it.
• The information you enter into a form will not be saved unless you complete each section entirely and submit the information. However, if you leave your web browser open, the information will not be lost before you return to it at a later point in time. You can RESET all of the data fields by clicking on the RESET button at the end of each form.

Since the surveys are somewhat long, we have split each into four sections so you do not have to complete the entire survey in one sitting. Please go to the appropriate questionnaire as listed below.
