Duplicate Tax Statement Request Form

Tax statements for the previous year should be available on the myUWM portal at (my.wisconsin.edu) after approximately February 4th. You can log in with your ePanther ID and password and print duplicate tax statements from the My Page tab. Requests for duplicate tax forms will not be processed before February 15th.
